Support at Home

(Previously known as Home Care Packages)

Support for older people with complex care needs to live independently in their own homes.

What we do

The Support at Home program provides tailor made support to assist elderly people to live at home.  A Care Partner completes an assessment with you and then develops a goal directed care plan.

Services can be provided to assist you to meet your goals. Some services that may be provided include:

  • Help with household tasks
  • Equipment (such as walking frames)
  • Minor home modifications
  • Personal care
  • Clinical care such as nursing, allied health and physiotherapy services.

Who can use our service?

Anyone aged 65 years and over, or 50 years and over if identifying as Aboriginal or Torres Strait Islander, who has been assessed by My Aged Care and has a referral code for Support at Home funding. Currently South Gippsland Hospital does not have a waiting list for entry into this program.

How you access

To find out if you are eligible visit Helping you navigate aged care, every step of the way on the My Aged Care website or call My Aged Care on 1800 200 422. You will be referred for a comprehensive assessment by the Aged Care Assessment Service. If you have been approved for an Aged Care Package you receive a referral code in writing (via letter) once the package has been assigned. The referral code is used by South Gippsland Hospital to provide your services.

Are there any fees?

Support at home fees are determined by a combination of your service type, income, and assets. You will pay a contribution towards some services, with the amount varying based on your financial situation and the type of support. While some services, like clinical care, are fully covered by the government, others require a percentage of the cost to be paid by you.
