New Employees – Onboarding requirements
Please print, review and sign forms below and supply in hard copy to the HR department or email HR@sghs.com.au prior to your first shift.
Pre-employment forms to download, complete and return:
- Staff Member Personal Details Form 464 (Word)
- Staff Banking Details Form 3 (Word)
- Health Immunisation History Form 1068 (Word)
- Workcover pre-existing injury declaration Form 22 (Word)
- Confidentiality Agreement Form 9 (Word)
- Driver’s Licence Registration Form 7 (Word)
- Swipe Card ID / Name Tag / Stamp Request Form 1391 (Word)
- Staff Timesheet Form 1142 (Word)
- Superannuation fund details Form 1133 (Word) - only use this form if your Super is with HESTA or AWARE (otherwise ATO Super form below)
- ATO Superannuation Standard Choice Form (PDF, opens new tab)
- ATO Tax File Number Declaration Form (PDF, opens new tab)
- Visit the Salary Packaging Information & application forms – for permanent part time employees website
Pre-employment documentation required to be provided:
- Copy of Drivers licence
- Copy of qualification/s required for position
- Copy of Medicare Immunisation History Statement
- Certificate of Service, where applicable
- Current Working with Children Check (see below links re application/update details)
- Current Police Check (issued within previous three months) (or see below links for new application
Pre-employment checks required to be provided:
- Police Check – you can obtain your own, from an alternate provider, or visit the CrimCheck website
- Working with Children Check – visit the Working with Children website to apply
- Working with Children Check – visit the Working With Children website to update your details/employee/list employer etc
