New Employees – Onboarding requirements

Please print, review and sign forms below and supply in hard copy to the HR department or email HR@sghs.com.au prior to your first shift.

Pre-employment documentation required to be provided:

  • Copy of Drivers licence
  • Copy of qualification/s required for position
  • Copy of Medicare Immunisation History Statement
  • Certificate of Service, where applicable
  • Current Working with Children Check (see below links re application/update details)
  • Current Police Check (issued within previous three months) (or see below links for new application
