If you can please print, review and sign forms below. Please supply in hard to HR department or email HR@sghs.com.au prior to your first shift.
Pre-employment forms:

- Staff Member Personal Details Form 464
- Staff Banking Details Form 3
- Health Immunisation History Form 1068
- Workcover pre-existing injury declaration Form 22
- Confidentiality Agreement Form 9
- Driver’s Licence Registration Form 7
- Swipe Card ID/Name Tag/Stamp Request Form 1391
- Staff Timesheet Form 1142
- Superannuation fund details Form 1133- only use this form if your Super is with HESTA or AWARE (otherwise ATO Super form below)
- ATO Superannuation Standard Choice Form
- ATO Tax File Number Declaration Form
- Salary Packaging Information & application forms – Permanent part time employees
Pre-Employment documentation required to be provided:
- Copy of Drivers licence
- Copy of qualification/s required for position
- Copy of Medicare Immunisation History Statement
- Certificate of Service, where applicable
- Current Working with Children Check (see below links re application/update details)
- Current Police Check (issued within previous three months) (or see below links for new application)
Pre-Employment Checks – required to be provided:
