What we do

Our Diabetes Education service provides personalised education and support to people living with diabetes to achieve optimal health and wellbeing. Clinics are run on a Monday and Thursday

How you access

Family or self-referrals are welcome, or clients may be referred by their GP or other health professional such as an Endocrinologist.

For further information or to book an appointment please contact our community care reception team Monday to Friday between 9am and 5pm on 5683 9780.

• Via a TCA which is claimed via Medicare (you will need approval from your GP for this).

Who can use our service?

The Diabetes Education service accepts referrals for any clients over the age of 18 years living with Type 1, Type 2 or gestational diabetes. South Gippsland Hospital can setup and provide ongoing support and education for eligible clients wishing to utilise Continuous Glucose Monitoring technology with either Freestyle Libre 2 or Dexcom G6. Family members, friends, carers and or partners of people living with diabetes are welcome to attend the clinic.

*Please note, we are unable to provide services to clients under the age of 18 years old.

Are there any fees?

There is a small fee associated with this service.

• Blood glucose monitor or record of your blood glucose readings
• Any referral letters you have from you Doctor
• Medication list
• Blood pathology results
• Hearing aids
• Reading glasses
• Any newly prescribed injectable medications
