Home Care Packages

South Gippsland Hospital (SGH) offers Home Care Packages (HCP) on a consumer directed care (CDC) model of coordinated services tailored to meet a person’s specific care needs that aims to assist them to remain living at home, and support them to have dignity, choice and flexibility in the way that their supports and services are provided.

SGH can provide HCPs at the following levels:

  • Home Care Level 1 – A package to support people with basic care needs
  • Home Care Level 2 – A package to support people with low level care need.
  • Home Care Level 3 – A package to support people with intermediate care needs
  • Home Care Level 4 – A package to support people with high care needs

HCP are funded by the Commonwealth Government. In order to be eligible for a HCP the care recipient must have been assessed and approved by the Aged Care Assessment Service (ACAS) and placed in the National queue.

Clients will be notified by My Aged Care (MAC) when they have been assigned a HCP.
