The Support at Home program supports older people with complex care needs to live independently in their own homes.
What we do
The Support at Home program provides tailor made supports to assist the older person to live at home. A Care Partner completes an assessment with you and then develops a goal directed care plan. Services can be provided to assist you to meet your goals. Some services that may be provided include:
- Help with household tasks
- Equipment (such as walking frames)
- Minor home modifications
- Personal care
- Clinical care such as nursing, allied health and physiotherapy services.
How you access
To find out if your eligible please call My Aged Care on 1800 200 422. You will be referred for a comprehensive assessment by the Aged Care Assessment Service. If you have been approved for a Support at Home Package you receive a referral code in writing (via letter) once the package has been assigned. The referral code is used by South Gippsland Hospital Support at Home to provide your services.
Who can use our service?
Anyone aged 65 years and over (or 50 years and over if identifying as Aboriginal or Torres Strait Islander) who has been assessed by My Aged Care and has a referral code for a Support at Home Package. Currently South Gippsland Hospital does not have a waiting list for entry into this program.
Are there any fees?
An income assessment will need to be completed by Services Australia.
