District Nursing and Home Based Care

The District Nursing Service is nursing service providing care in your home. South Gippsland Hospital offers a daily service, between 8am and 4.30pm Monday to Friday and 8am to 12 noon on Saturday and Sunday.

Patients can be referred by nursing or medical staff or self-refer.

Fees

A client co-contribution amount is charged according to your ability to pay, with a small fee charged for pensioners and health care card holders. An additional fee for wound care dressings may also be charged.

You may be eligible for support to access the District Nursing Service via:

  • Department of Veteran Affairs (DVA)
  • Traffic Accident Commission (TAC)
  • Home Care Packages
  • Post-Acute Care (PAC)
  • Hospital in the Home (HITH) and/or
  • Palliative Care funding

Weekly Clinic

The DNS also provide a weekly Wednesday clinic at the Community Health Centre for patients who are more independent and who require general nursing care such wound care, vital signs, foot care, catheter changes and other health related requirements.

Stoma and Continence Clinic

A Registered Nurse is available to provide education and advice regarding:

  • bladder and bowel problems
  • stomal care
  • products for ongoing health care
  • assistance with accessing funding sources
