Position Description

South Gippsland Hospital (SGH) invites the appropriately skilled and experienced Nursing professionals, to apply for the above position.

As a member of the nursing leadership team, this role is responsible for the effective coordination and management of Community Nursing Services including, but not limited to, District Nursing, Diabetes Education, Sexual and Reproductive Health, Adolescent Health and Chronic Disease Management.
The role will support acute nursing and other services to ensure seamless and coordinated patient care.

The role will require demonstrated:
• leadership and management within a community nursing setting or similar,
• commitment to SGH organisational values,
• financial management and reporting of community nursing services budgets,
• ability to provide workforce planning, rostering, recruitment and retention in all community nursing areas,
• evidence of professional development,
• commitment to quality improvement and patient safety and
• current Victorian driving licence, successful police and Working with Children’s Check.

Are you passionate about aged care? Can you demonstrate knowledge of, and commitment towards, supporting frail aged people to remain living at home? Do you enjoy getting out and about and supporting others to do so? Do you want to work with a supportive and caring team?

A vacancy exists for a casual worker to assist clients within our Home Care Package program, helping clients to remain living safely at home in accordance with their individual goals and preferences.

Tasks for the role may include (but not limited to):

  • Provision of high quality, client directed services in the client’s home.
  • Assistance with daily living activities, personal hygiene tasks, home duties, social support and/or providing home respite.
  • Assistance with meal preparation and/or shopping.
  • Identifying and reporting situations that require assistance from other healthcare staff.
